State Approved Forms
Florida has adopted numerous forms that are used for Charter School Authorizing practices which may apply to the
work of the commission. The forms are being included here for reference.
Charter School Review Commission Forms
- IEPC-LOI - Letter of Intent for Commission Review - Administrative rule requires applications to submit a letter of intent prior to applying. The rule does not give a deadlines, the FCI has asked that applicants submit the letter three weeks prior to the application deadline.
- IEPC-INP – Standard Form for District School Board Input – This is the form that districts may use to submit their feedback to the commission on charter applications that are submitted to their districts.
Charter Application Forms
- IEPC-M1 – Model Florida Charter Application – This is the form that most charter schools use when applying.
- IEPC-M2 – Standard Model Charter School Application Evaluation Instrument – This is the form that will be used to evaluate the model charter application, and what the FCI will use to provide feedback on applications to the Department.
- IEPC-V1 – Virtual Charter School Application – The form that is used to apply to start a virtual school, it is very similar to IEPC-M1 but excludes Facilities, Transportation, Food Service, and School Safety sections.
- IEPC-V2 – Virtual Charter School Application Evaluation Instrument – The form used to evaluate virtual applications.
- IEPC-HPS1 – Model Florida Charter School Application High-Performing Charter School System Replication – The application for systems who are identified as high performing who wish to replicate. It is a much shorter version of a charter application.
- IEPC-HPS2 – Model Florida Charter School Application High-Performing Charter School System Replication Evaluation Instrument – The evaluation form used when form IEPC-HPS1 is submitted.
- IEPC-M1A – Applicant History Worksheet – This worksheet must be included by applicants who have history with other charter schools to identify which schools they are affiliated with.
Additional Reference Forms
- IEPC-SC – Florida Standard Charter Contract – While the commission will not be responsible for negotiating or participating in the contract process, it may be helpful for commission members to be aware of what is in the standard charter contract which is used as the starting point for the negotiation process.
